Health Information & Immunizations
New York State Public Health Law requires students to get certain vaccines to attend childcare or school. We encourage families to take time to review their children’s immunization history with their healthcare providers, so that they may be up-to-date for the start of the school year. You can find more information on the NYC Public Schools website: Immunizations (nyc.gov)

Any students needing to take medications or receive any medical treatments must submit a Medication Administration Form (MAF) and/or Medically Prescribed Treatment Form (scroll down on the page) every year.
